Join the PFMC Network
Membership in the PFMC network brings many benefits to physicians and other health care providers. We offer no-risk, fee-for-service contracts and reimburse our members at rates well above Medicare. Also, because of our affiliation with the California Foundation for Medical Care, our members belong to one of the largest PPOs in California. The combined PFMC/CFMC network features a large patient base and a variety of payers. All medical specialties are represented in the network, with no restrictions on referrals.
Because PFMC is a nonprofit entity governed by physicians, the network is designed to support our members rather than to squeeze them for profits. All members are automatically enrolled in our PPO network. We also offer an exclusive provider organization (EPO) network and a worker’s compensation organization (WCO) network. Physicians and other providers in the PFMC service area are welcome to apply for membership at any time. Our staff handles the entire credentialing process, and most applications can be approved within a matter of weeks.

Types of Networks
PFMC administers three types of provider networks: a preferred provider organization (PPO), an exclusive provider organization (EPO), and a worker’s compensation organization (WCO). Our PPO is part of the largest provider network in California and is used by a long list of health plans and self-insured employers. The EPO is a smaller network in which providers agree to greater discounts and expanded utilization review. The WCO is for worker’s compensation cases. All our providers are automatically enrolled in the PPO network and have the option of joining the EPO and WCO networks. Providers in the networks agree to accept PFMC’s fee schedule and to have their medical services reviewed by peers in their field. Providers also agree to bill on behalf of the patient and to accept the PFMC allowable as payment in full, less the patient’s responsibility for copayments, deductible, or coinsurance. There are no restrictions on in-network referrals.
Apply for Membership
Physicians and other health care providers in the following California counties are eligible for membership in PFMC: Alameda, Contra Costa, El Dorado, Imperial, Marin, Napa, Nevada, Placer, Sacramento, San Diego, Solano, Sonoma, and Yolo. Providers in other California counties can join similar networks run by their local medical foundations. For a complete list of foundations, see our PFMC Service Area map. The application process is straightforward. After you contact us, we send you an application form, which you can return via fax or mail. Our staff handles all credentialing, saving you the trouble of credentialing with multiple payers. Completed applications are submitted to our physician-led board of trustees for final approval.
