Pacific Foundation for Medical Care supports non-profit, health related projects that enhance the availability or quality of health services in communities. Since 1993, PFMC has provided more than $1.4 million in grant awards to 75 different organizations and programs in the North San Francisco Bay Area and San Diego County.
In Spring 2024, PFMC awarded a total of $45,000 to the following recipients:
CTE Foundation – Sonoma County
To provide program support for career-connected learning to high school students. The grant will support the expansion of the CTE programs throughout the county, increase student engagement with relevant hands-on learning opportunities, leading to the health and wellbeing of our students, their families and our community.
Imani Clinic – Sacramento County
For a new diabetes long-term care program to significantly impact the community. The endocrinology pilot program will test a new approach to managing chronic conditions like type 2 diabetes mellitus. The project will integrate Continuous Glucose Monitors (CGM) with the Imani Health Education and Outreach program to provide comprehensive support for patients with Type 2 Diabetes.
Jewish Community Free Clinic – Sonoma County
For general operating support of weekly free clinics with and all-volunteer staff that provides free medical care and social services to thousands of uninsured adults and children in need.
